A proposito del Sistema Saniario Nazionale

Il Sistema Sanitario Nazionale Italiano ha una lunga storia, e sebbene oggi sia qualcosa che tutti noi cittadini italiani diamo per scontato, rimane un organo molto fragile che va tutelato e protetto per il bene dell’intera comunità.

La prima idea embrionale di sistema sanitario pubblico in Italia si diffuse già sul finire dell’Ottocento, come un organismo di salute pubblica il cui scopo era quello di intervenire nel caso di epidemie. Ciò però non comportava completo aiuto medico alla popolazione perché all’epoca la Direzione Generale della Sanità Pubblica (istituita con la legge Pagliai nel 1888) era una branca del Ministero dell’Interno, quindi il suo scopo era legato al garantire l’ordine pubblico con zone di quarantena o la corretta rimozione dei cadaveri infetti. La cura delle persone era ancora tutta a carico del singolo. I ricchi potevano permettersi i medici, la classe lavoratrice si affidava al farmacista e i poveri potevano contare su una scarna rete di ospizi ed opere di carità. Le opere pie furono regolamentate con la legge Crispi del 1890, che le distinse dai servizio di pubblica assistenza.

Una novità rispetto al passato però cominciava a farsi strada e a inserirsi nei tessuti urbani: tra gli anni Sessanta ed Ottanta dell’Ottocento iniziarono a costituirsi sul territorio le prime SOMS (Società di Mutuo Soccorso). Non garantivano assistenza sanitaria completa, ma prevedevano delle indennità in caso di infortuni sul lavoro e supporto alle vedove e agli orfani.

A cavallo tra le due guerre mondiali la situazione iniziò però a cambiare. Il partito fascista, soprattutto per acquisire consenso tra la popolazione iniziò una serie di politiche volte al sociale, tra cui il consolidamento del sistema della “mutua”, ed il supporto all’opera di istituti sanitari volti alla lotta alla tubercolosi e alla malaria.

Sulla carta la mutua fascista sembrerebbe un’idea positiva, ma analizzando nel dettaglio il suo funzionamento ci si rende conto che non era un programma di copertura universale.

Ogni cassa della mutua era un’entità a se stante, specifica per la categoria di lavoratori, e nel tempo iniziarono a nascere un’enormità di queste società di previdenza sanitaria, come l’ENPAS (lavoratori pubblici dipendenti statali), l’INADEL (lavoratori pubblici dipendenti locali) o l’impronunciabile ENPDEDP (lavoratori del credito e delle assicurazioni).

Le casse mutua garantivano la copertura sanitaria all’iscritto e alla sua famiglia, finanziata dagli iscritti a quella particolare cassa, e questo poteva creare delle disparità nei trattamenti ricevuti. Casse ricche avrebbero garantito cure migliori ai propri assistiti, i quali dovevano essere legati con un vincolo di parentela al lavoratore, lasciando scoperte le persone che per qualche motivo erano inabili al lavoro. Inoltre non era prevista alcuna tutela durante il periodo di riabilitazione o a prassi preventive.

Per mettere ordine in questa giungla di casse mutua cresciute a dismisura lo stesso Mussolini durante la Seconda Guerra Mondiale istituì con il regio decreto 138 dell'11 gennaio 1943 l’Ente di Mutualità Fascista, che raccorpava in un unico ente nazionale le varie casse mutua.

Alla caduta del regime
l’EMF fu ribattezzato INAM (Istituto Nazionale per l'Assicurazione contro le Malattie) con il D. Lgs 435 del Capo Provvisorio dello Stato il 13/05/1947.
Inoltre, nel 1958 con la legge n. 296, viene fondato il Mineistero della Sanità, indipendente ed autonomo da quello dell’Interno.

L’INAM cessò di esistere nel 1977 con la legge 833 che istituì il Sistema Sanitario Nazionale (SSN), entrata in vigore a partire dal 1/07/1980. Questo importantissimo Ente ha rivoluzionato l’accesso alle cure mediche per la popolazione italiana: la salute finalmente diventò un diritto di tutti, indipendentemente dalle condizioni sociali.

La salute è un bene necessario per
l’equità tra le persone, in quanto non si sa che cosa la vita abbia in serbo per ciascuno di noi, e nonostante le tante imperfezioni e deficit del SSN, noi italiani possiamo ancora contare su un accesso a contributo minimo o anche totalmente gratuito alle cure, a cui non importa delle patologie pregresse, asset genetico o conto in banca. E che alla fine è nel 2018 si è classificato quarto a livello mondiale (fonti Bloomberg). Utilizziamolo con saggezza senza abusarne, perché al Mondo esistono Stati più avanzati della piccola Italia che sognano il giorno in cui l’assistenza sanitaria sarà diritto universale.

The Italian National Health System has a long history, and although today it is something that all of us Italian citizens take for granted, it remains a very fragile public service that must be protected for the good of the entire community.

The first embryonic idea of a public health system in Italy spread as early as the late nineteenth century, as a public health organization whose purpose was to intervene
during epidemics. This however did not involve complete medical aid to the population because at the time the General Directorate of Public Health (established with the Pagliai law in 1888) was a branch of the Ministry of the Interior, therefore its purpose was linked to guaranteeing public order with quarantine areas or the correct removal of infected corpses. Health care was still entirely up to the individual. The rich could afford doctors, the working class relied on the pharmacist and the poor could count on a meager network of hospices and charitable works. The pious works were regulated by the Crispi law of 1890, which distinguished them from the public assistance service.

A novelty compared to the past, however, was beginning to make its way and to insert itself in urban fabrics: between the sixties and the eighties of the nineteenth century the first SOMS (Mutual Aid Society) began to be established in the territory. They did not guarantee full health care, but they provided compensation in the event of accidents at work and support for widows and orphans.

At the turn of the two world wars, however, the situation began to change. The fascist party, especially to gain consensus among the population, began a series of social policies, including the consolidation of the "mutua" system, and support for the work of health institutions aimed at combating tuberculosis and malaria.

On paper, the fascist mutua s
ystem would seem a positive idea, but analyzing in detail its functioning, we realize that it was not a universal coverage program.

Each mutua fund was an entity in its own right, specific to the category of workers, and over time an enormity of these “
mutuas” began to emerge, such as ENPAS (state-employed public workers), INADEL ( local public employees) or the unpronounceable ENPDEDP (credit and insurance workers).

The mutua funds guaranteed health coverage for the
worker and his family, financed by the members of that particular fund, and this could create disparities in the treatments received. Wealthy mutua funds would have guaranteed better care to their members, who had to be tied with a relationship of kinship to the worker, leaving people who for some reason unable to work uncovered. Furthermore, there was no protection during the rehabilitation period or preventive practices.

To bring order to this jungle of mutual funds that had grown enormously,
the dictator Mussolini established the Fascist Body of Mutuality with the Royal Decree 138 of 11 January 1943, which brought together the various mutual funds into a single national body.

When the regime fell, the EMF was renamed INAM (National Institute for Health
Assistance) with Legislative Decree 435 of the Provisional Head of the State on 13/05/1947.
Furthermore, in 1958 with the law n. 296, the Ministry of Health is founded, independent and autonomous from that of the Interior.

INAM ceased to exist in 1977 with law 833 which established the National Health System (
Sistema Sanitario Nazionale or SSN), which came into force on 1/07/1980. This very important body has revolutionized access to medical care for the Italian population: health has finally become a right for everyone, regardless of social conditions.

Health is a necessary good for equity among people, as it is not known what life has in store for each of us, and despite the many imperfections and deficits of the
SSN, we Italians can still count on access to totally free treatments, which does not care about past pathologies, genetic assets or bank account. And last but not least in 2018 the SSN ranked fourth worldwide (sources Bloomberg). Let's use it wisely without abusing it, because in the world there are more advanced states than small Italy that dream of the day when healthcare will be a universal right.
